I know I know. Cop out with the movies, but it's what I know well. And I'm pretty sure most movies are made into books as well, so...well...movies.
Disease: Outbreak
Mutation: X-Men
Survival: Cast Away
Disease and Mutation: Mask (not the one with Jim Carrey)

That was way harder than I thought it would be. It is hard to separate disease and survival. Also, disease and mutation really had me thinking.
This one is pretty tough. I can only think of enough answers with other category as large as movies: video games.

Thank goodness for the survival horror genre of game, otherwise the Survival and the All categories probably wouldn't have had anything in them.
